        No blasts or promyelocytes in peripheral blood 
        Bone marrow blasts ≤ 5% 
        Less than 5% myelocytes plus metamyelocytes in peripheral blood 
        Basophils in peripheral blood < 2% and basophils in the bone marrow < 2% 
        No extramedullary involvement (including no hepatomegaly or splenomegaly) 
    No evidence of leukemia (NEL) 
        WBC count no more than institutional ULN 
        No blasts or promyelocytes in peripheral blood 
        Bone marrow blasts ≤ 5%
