Hypercalcemia
    serum calcium > 0.25 mmol/L (1 mg/dL) above the upper limits of normal or > 2.75 mmol/L (11 mg/dL) 
Renal insufficiency
    serum creatinine > 173 mmol/L (1.96 mg/dL) 
Anemia
    Hgb 2 g/dL below lower limits of normal or Hgb < 10 g/dL 
Bone lesions
    lytic lesions or osteopenia with compression fractures 
Other
    symptomatic hyperviscosity, amyloidosis, recurrent bacterial infections (> 2 episodes in 12 months)
